Patient Flow Facilitator job overview
An exciting opportunity has arisen within the Capacity Centre for a Patient Journey Facilitator.
Patient Journey facilitators are ward based and focussed on delivering the SAFER Red2Green approach to patient care. SAFER Red2Green aims to value patient time and improve flow by reducing delays in a patient’s hospital journey.
We are therefore seeking a keen individual with a ‘can-do’ attitude who is able to work alongside the clinical teams to progress the patient’s journey. The successful candidate will need to be proactive and personable with a strong understanding of SAFER Red2Green and an appreciation for the importance of its principles.
There are two positions available:
1 x full time post (37.5 hours per week, Monday to Saturday)
1 x part time (30 hours per week, Monday - Saturday)
